Heart care for Clay County seniors

FCCI cares for many older adults in Clay County and accepts Medicare and Tricare for Life. Caregivers are welcome at visits. Call 904.644.0092.

Why heart checks matter after 65

Risk for heart disease, valve problems, AFib and circulation trouble goes up with age. Symptoms can also be quieter in older adults: more tiredness, less energy, or getting winded on stairs. Those changes are worth a visit.

FCCI accepts Medicare, AARP Medicare Complete, Tricare for Life and many Medicare plans. Call 904.644.0092 to confirm yours.

Do not write off new symptoms as just getting older. A short visit can tell you whether something needs attention.

Bring a buddy

A family member or friend can help you remember questions and take notes. Bring a full list of medicines, including over-the-counter pills and vitamins. If you use a cane or wheelchair, call ahead and the front desk can help you plan your arrival.

It also helps to bring a short list of questions. Good ones include: what is this test for, what are my numbers, what should I watch for at home, and when should I come back? Ask the provider to write down anything you want to remember. Questions about any of this? Call 904.644.0092 and talk with a real person at the Orange Park front desk, Monday to Friday, 8 a.m. to 5 p.m.

Help for caregivers

  • Write down symptoms with dates and times
  • Keep a simple blood pressure log if the doctor asks for one
  • Watch for new swelling, weight gain or shortness of breath
  • Know the 911 signs: chest pain, fainting, sudden weakness or trouble speaking

Caregivers, please do not wait and watch. If your loved one has chest pain or pressure, sudden shortness of breath, faints, or shows stroke signs like a drooping face, a weak arm or trouble speaking, call 911. Do not put them in the car yourself, and do not wait for their next appointment.

Visits that fit your day

FCCI's offices are open Monday to Friday, 8 a.m. to 5 p.m. Some visits can be done as a TeleVisit: you get a text link and talk with your provider by video, with no app to download. Ask if that fits your appointment.

TeleVisits work on a smartphone, tablet, laptop or desktop with good internet. A family member can help you set up the call from home. In-person visits are still best for exams and tests, and the front desk can help you choose. Questions about any of this? Call 904.644.0092 and talk with a real person at the Orange Park front desk, Monday to Friday, 8 a.m. to 5 p.m.
